2023 Benefits Open Enrollment

The 2023 Florida Blue medical insurance open enrollment started Monday, Oct. 24, and continues until midnight on Friday, Nov. 4.

CF will continue to cover 100% of each eligible employee’s cost of health insurance. This information is also available on the CF Intranet at this link: 2023 Health Plan Open Enrollment.

1. Enrollment is MANDATORY for 2023 as a result of the new health plans. If no action is taken, you will be defaulted into the PPO Gold Plan with Employee-Only coverage; all other current benefits will roll over to the new plan year.
2. Plan information, rates and enrollment instructions are located at this link: 2023 Health Plan Open Enrollment.
3. HR has provided an informative benefits presentation, including a health plan informational recorded presentation, for employees to view at their own convenience on the Employee Benefits web page.

Health, Life, Dental, and Vision Enrollment
Enroll via FBMC online benefits portal. FBMC enrollment instructions can be found on the Employee Benefits web page, available at this link: Employee Benefits

Health Savings Account Contributions
New or existing HSA health plan members wishing to contribute additional funds to your HSA account via payroll deduction must complete a new election form HSA Payroll Deduction, available at this link: HSA Payroll Deduction

Adding NEW Dependents this Year?
If you elect to add new dependents to insurance (health, dental, vision), you will be required to provide documentation showing that they meet eligibility requirements. To verify your dependents, you must submit the required documentation as defined in the Dependent Verification Documentation Chart along with the signed Certificate of Dependent Eligibility Attestation. Required documentation must be received in Human Resources, or uploaded to the FBMC “Document Upload” feature, by close of business on Nov. 4, 2022, for the Jan. 1, 2023, effective date of coverage. Your enrollment will remain in “pending” status until required documents are received in HR.

Requirements for Enrolling in Supplemental Life Insurance
An Evidence of Insurability Form is required for any increase resulting from a plan or option change that employees elect. Evidence of Insurability is not required for your spouse or child(ren). You can submit an Evidence of Insurability in one of two ways:

Option #1: Complete an Evidence of Insurability Form by Nov. 4, 2022 and submit to HR.

Option #2 (PREFERRED METHOD): Electronically submit the form directly to the Standard and notify HR.

Short term Disability, Long Term Disability, Cancer Insurance, or Medi-Gap Plan Enrollment
First time enrollees: Contact Cornerstone Benefits Services representatives at 352-369-9453 or email tom@cornerstonebenefitsfl.com.

Current enrollees do not have to re-enroll unless you are making changes.

Flexible Spending Account Enrollment
Members must re-enroll to make new monetary elections each year. First-time enrollees will need to submit a paper enrollment form. Click this link for the form: Eagles Benefit Form

Sick Leave Pool Open Enrollment

ENROLLMENT ASSISTANCE

Here’s a quick list of action items as you prepare for open enrollment:

  • Visit representatives on campus to learn more about voluntary benefits (dental, vision, long-term/short-term disability, cancer coverage, hospital supplement (GAP coverage)
    • Ocala Campus: Mon., Oct. 31, Founders Hall Conference Room, Room 105, from 11:30 a.m.-1:30 p.m.
