The brochure and form are available ONLINE ONLY and can be found on the Foundation website. Completed forms must be submitted to the Sharon Reyes in the Foundation office, located on the Ocala Campus in the Enterprise Center, Building 42, Room 202 or via email at reyessh@cf.edu, by 4:30 p.m., Friday, March 7.
