Reminder for 2019 Flexible Spending Account Participants

All Flexible Spending Account out-of-pocket claims for the 2019 previous plan year must be received no later than April 29, 2020. If a claim is received after this date, according to IRS regulations, it will not be considered for reimbursement.

Claims may be submitted through the following methods:

• Mobile device: MyFlex app
• Fax: 352-291-6690 or 352-369-9453
• Email: custombenserv@msn.com
• Postal mail

Forms of eligible documentation:
• Itemized provider’s receipt
• Insurance Explanation of Benefits may be submitted.

Information required to determine eligibility:
• Provider name and address
• Service date
• Services rendered
• Charges or patient responsibility
• Some services rendered/ expenses may require a doctor’s letter of medical necessity
