To help improve the long-term health and quality of life for employees enrolled in the State Group Health Insurance Plans, the Department of Management Services (Department) will offer the Diabetes Management Pilot Program and the Weight Management Pilot Program for the 2026 plan year. The Division of State Group Insurance (DSGI) administers these programs and asks that you distribute this advisory to all employees.

Application Process
There will be a combined application for the DMP and WMP for 2026.
The application period for the 2026 programs will be November 3, 2025-November 17, 2025.
- Download the 2026 Provider Referral Form.
- The applicant’s provider completes and signs the referral form.
- The applicant signs the referral form.
- The applicant submits the referral form with the online application through the link on the WMP or DMP websites. The link will only be open November 3, 2025 – November 17, 2025.
NOTE: The Department will only review applications and Provider Referral Forms sent through the links.
Only the combined results of all participants will be reported to the Florida Legislature. Information about individual participation will not be disclosed.
